What to Bring with You to your Physical

Please bring the following if they apply to you:

  • A valid driver’s license
  • Glasses or contacts you normally wear while driving
  • Hearing aids, if applicable
  • List of current medications/supplements
  •  Any records of medical conditions
  • List of any allergies
  • If a driver has any pre-existing conditions, documentation from their Primary Care Physician is necessary at the time of the DOT physical exam.

How to Prepare for your DOT Physical Exam

  • Fill out forms ahead of time. You can cut down time in the office by following the link for your forms. https://form.jotform.com/231576199645065
  • Make sure you have all documentation you may need from your Primary Care Physician (PCP).
  •  Healthy Eating! Cutting down on caffeine, salt, & sugar are recommended to maintain a healthy lifestyle & prepare for the DOT physical exam. This may affect your blood pressure when you come in for your physical.
  • Hydrate! We will need a urine sample so ensuring you’re hydrated before you come into the office is important.

What to Expect at your DOT Physical

To get you back on the road & moving on with your day, Dr. Heyer will have you in and out of the office within an hour!

The physical exam will consist of:

  1. Medical History
    • It is important that each driver provides an honest background of their medical history to Dr. Heyer, including past surgeries, medications, and any known health conditions.
  2. Vitals and Measurements
    • Dr. Heyer will go through blood pressure, weight, height, BMI.
  3. Hearing Check
    • Each driver is required to pass a “Whisper Test.” Dr. Heyer will stand 8-5 feet away from the driver and whisper. The driver will then need to repeat what they heard. If the driver is unable to pass this, then Dr. Heyer needs to send them to an audiologist.
  4. Vision Testing
    • The FMSCA requires drivers to meet vision test standards. These standards are at least 20/40 vision acuity in each eye with or without correction (glasses or contacts) and at least 70-degree field of vision in horizontal meridian measured in each eye (the area of visibility for each eye’s line of sight). Drivers also need to pass a color test ensuring they can distinguish among traffic control signals and devices showing red, green, and amber colors.
  5. Urinalysis
    • Dr. Heyer is required to do a urinalysis to rule out any underlying conditions. This looks at the protein, blood, and sugar in a urine sample.
  6. Medication Check
    • Dr. Heyer will review all medications that the driver is taking. Some medications may interfere with safety and disqualify you from obtaining your medical card. If you are on any medications that may interfere, if your Primary Care Provider is able to provide a letter stating the medication will not affect your ability to drive, you may still be able to receive your medical card.

We do not provide in-house drug & alcohol testing.

What Happens Next?

Upon finishing the exam, Dr. Heyer will complete the assessment and proper paperwork. He will certify the driver for the full two-year period, or if medically necessary, shorter. Certain health conditions require a shorter amount of time between DOT physical exams. If Dr. Heyer feels like he does not have enough information at the time of the physical exam, approval will not be granted, and the exam may be suspended until further information is gathered.

Once approved, Dr. Heyer will give the driver a signed certification card at the end of the visit. Drivers must carry this with them at all times while they are on the road.

Frequently Asked Questions

According to the Federal Motor Carrier Safety Administration (FMCSA), the governing group of The Department of Transportation (DOT), requires a driver operating a commercial vehicle to go through a physical exam to ensure they are healthy enough to safely operate the vehicle.

Depending on the outcome of the physical exam, a certification can be valid for up to two years. However, if there are certain health conditions, Dr. Heyer may determine that they need to be monitored and give less time. Common conditions include high blood pressure, heart disease, diabetes, or a sleep disorder/apnea.

Typically, Dr. Heyer gets drivers in and out of the office within the hour.
